It's a terrible thing I agree, but such is the level of violence and mayhem in many places across America (not bashing America, just stating the truth), that this only rates as a 'mere' "terrible" to this official.
The question was asked of the spokesman at the end of a wide-ranging daily press briefing. No follow-up was asked. It is hardly as if this is any sort of definitive response.
QUESTION: Any reaction on the Tokyo stabbing incident again, or the comment really on camera?

Terrible indeed because it happened in Japan, a peaceful country that is known for low crime rate.

At least he didn't call it a 'good thing'.

On a scale of 1 to 10, how "terrible" would you say that this even was, Mr. State Department spokesman?

If it were reversed I'm sure a Japanese foreign ministry spokesman would call it a 'regrettable' thing.
It was indeed a terrible thing for all involved, including any good Samaritan who know might have hepatitis.

Why should his sentiment be more sympathetic than the Japanese?

I am not sure what some commentators here expected him to say or do. Would ritual suicide satisfy?
