Is it for boys or girls to answer? Mistakes are part of experience that make each individual perfect at later stage, as such making mistakes may be a necessary evil.
Don't talk about past relationships, don't make small talk, don't ogle passing women or men. Avoid going to a movie on a first date because you don't know each other yet. You sit there during the movie trying to mentally prepare conversation for wherever you go afterwards. Movies are something you can enjoy together after you get to know each other.
I prefer a first date to be something with a limited time, maybe lunch. That way, if you don't feel any chemistry, you can finish quickly.
Finally, don't expect (or even desire) the first date to end up in bed. That removes a lot of anxiety beforehand. Personally, I think sex on the first date is not a good way to start a long-term relationship.
Don’t take her to a wicked expensive restaurant, you set the bar a bit too high to soon. And don’t eat anything that can get stuck in your teeth (big turn off). Oh yeah and wear a clean diaper.
Do not carry less than 100 thousand yen in your wallet.
Do not show a credit card which is not gold or platinum.
Do not appear with a hairdo which is not the latest trend, even if it makes you look like an anime character.
Do not, god forbid, make a mistake about details of brand bags.
Do not forget to teach her some English over the drinks, that´s why she is dating you: because you are an English native speaker.
The last time this topic came up I advised people against sucking their contact lenses and/or removing their false teeth and cleaning them in the provided glasses of water. Nothing has changed.
I think it's important to remember that what goes down well elsewhere, is not a guaranteed winner in a romantic setting. Lighting one's farts, for instance, is unlikely to win the heart of a lady outside the Liverpool area.
Talking too much, constantly asking "are you okay?", talking repeatedly of an ex, or others that you've dated before, and... oh yeah, leaving your bottle of ruffies on the dashboard (they hate that).
Chivalry and chauvinism are words that sound similar, and this country has a helluva lot of both. That means if you're a decent guy, you pay on the first date. If your date is a decent lady and your financial circumstances are similar, she will offer to split the bill or pay alternately subsequently.
No wonder my Japanese female friends grumble about gaijin guys being cheapskates...

Don't be late, don't be too early.
Be yourself.

Just be yourself and be comfortable but stay away from asking too many personal questions or you may scare your date away.

Forgetting to take off your wedding ring.

One would like to think that boys and girls are playing, just like children should be and that men and women are the only ones dating.

Just ask questions. Women only like to talk about themselves and they dont give a s$1t what you have to say.

It's always a mistake to give your real name and telephone number.

Don't be yourself, be someone more interesting, like a pro footballer, Goldman Sachs banker or a volunteer at an orphanage.

Offering to pay for her is a huge mistake.

Fairyprince,
Do you mean offering to pay for her dinner, or offering her cash to perform certain sordid services?

Offering to pay for her dinner/movie/drinks etc.

Failing to read her deepest hidden feelings.

Expectations. Just let things flow naturally. Not expecting the person to be a certain way, or making more of the date than what it is.

"Offering to pay for her is a huge mistake".
In this day and age, I couldn't agree more.
The other big mistake is forgetting your date's name. Also forgetting your own name is quite bad too I've heard.

Believing anything a Japanese woman says.

Asking her about her financial status, trying to offer her solutions to her problems, rushing her to the bed..
Talking about yourself too much, smelling of smoke and liqour, sweat..
Just treat her wit respect and be genuine in your praise..look her in her eyes and smile happily thats the best way.

Could this be the reason I am now married to one? Why didn’t you tell me this sooner?

Please, please don’t tell me that is the voice of experience speaking?
What are some first-date mistakes to avoid?
Proposing marriage.

Getting too touchy and immediately text or email a love poem telling her you are committed to her. Doing all the talking! Such a put off.

Already having the condom on, although it exudes confidence, it might sent the girl the wrong message!

What, did you strap it on before you left for the resturant, or something?

Three rules for dating Japanese chicks: 1. Don't fall in love 2. Believe nothing 3. Get them to pay (optional)

Expecting the girl to go dutch if she never indicated she wanted to. If she indicates, he should still insist on footing the bill alone.

sleeping on the first date is a big no-no for me.

Never tell a j-girl you want to go Dutch. She'll show up with an inflatable doll.

Nothing says confidence more than dozing off during a date. Women love confidence.
