Another idea would be to link participation in the national healthcare scheme (by hospitals) to their willingness to treat patients. Let's face it, if everyday patients could not use their health card at their local hospital (and instead would be forced to pay the full price of any treatiment), hospitals would go out of business.
Press charges if the patient is harmed by the delay in treatment. Hold administration and doctors responsible. If a doctor goes to jail for refusing to treat a patient, then the shame and disgrace should convince others to do better.
In the US, it is illegal for a hospital emergency room to refuse care to a patient. The priority is on the most serious injuries. Minor things can have waits of several hours, but a serious issue is treated immediately no matter what. (Although there is plenty the US health care system could learn from Japan, too. No system is perfect.)
Japan needs more doctors and health care staffs to meet the demand. US gets young, competitive doctors from all around the world that is keeping the health services in US running. Same in Australia and UK. Japan has not been able to do so because of certain barriers. State of Indonesian nurses who have been called to help Japan is being discussed in the news recently.
Forget health insurance for workers only but have a subsidized health system like France/Canada/UK etc. Get rid of the cost overruns.
Open clinics so people with colds and hayfever don't tie up emergency wards in hospitals. The lack of a family doctor, clinic, hospital trifecta system continues to hamper any improvements.
Oh, and make dental care a part of the general health system. Another waste. Canada has a healthcare system but dental isn't included, with obvious results.

And hell, the emergency wards are crowded with old people bringing a mosquito bite.They are usually loud, and they usually go there on a monthly basis, and they have aaaaallll the time in history. Japan should find a way to make japanese people not going to the hospital for a cold or flu. Waste of time, energy and money.
This is really a logistics problem. If private enterprise is not willing to solve it the Gov should set up the program. A centralized program needs to be setup to know what Doctors are available at any given time.
A centralized entity should be giving the emergency crews guidance to what services are available.

Of course, with a large aging population, and the propensity for old people to go to the hospital for every minor thing, this would require them to take a back seat, and that ain't gonna happen any time soon.
Sadly, the solution to many Japanese problems lies at a deep societal level, and won't be coming anytime soon.

It's a market issue. If hospitals don't have enough doctors on had, they cannot take more patients.

I think the doctors are afraid of liability...afraid with a possible mistake...!
What could i tell...? "Dont be a doctor" or "get another occupation"...?
Something strange is going on inside the hospitals...!
The law has to be stronger for hospitals administration and not with the doctors...!
