Singapore is head and shoulders above anything else I have ever flown on. Not just for the appearance of attendants, but the service, general comfort (economy on Singapore is like Business on Qantas or other airlines) ANA was pretty good as well. I have heard good things about Emirates and Virgin, however.
As for bad airlines - Lufthansa, Qantas, Continental, Malaysian all stand out to me as sucking. I've probably used Qantas more than any other airline, and now that I have seen what good quality service is on other airlines, can't stand the rudeness and laziness of flight staff, crap food, and most of all, the utter inflexibility and incompetence of ground staff. I don't know how many times I have seen passengers forced to open their suitcases at the check-in and arrange their suitcases for being 1kg over (because union bag handlers won't carry them). Also had a bad experience of having had changeover boarding passes misprinted which has made me miss flights.
Gotta agree with Hiko. Singapore (SQ) is the best in the sky, Hardware and software wise. I also agree that I like ANA (NH)for its service. The F and C class service is awesome with ANA. I've also had just as good experiences with JAL (JA) on F and C classes, but their Y class service loses to ANA. Cathay (CX) is also good with its Marco Polo service.
I can tolerate Continental and AA in the trans-pacific routes only on C class or higher but I think they are waaaaay below the Asian carriers in service and the overall quality of the hardware. Some AA 777's are downright filthy in the inside. Northworst and UA are out of the question, service wise.
Final point, Asian carriers have much better (younger and prettier) personnel compared to North American carriers.

i flew qantas once in 1997 and never will again - food was cold (thai curry), the FAs are obnoxious - one trying to speak japanese was a joke - offensive too....Korean is terrible - i asked for a glass of water halfway though a 12hr flight and was told there was no more water left on the plane!!!!! Another FA came past and got me a bottle immediately...

Air France's cabin crew were really helpful. But their ground staff were rude, unhelpful and generally lazy. A late domestic flight meant we missed our Japan-bound flight. They couldn't be half-arsed to get our luggage. Never again with them.
Once on United, a male flight attendent went up to a kid, took some of his potato chips without asking, ate it, smiled at the kid, and walked off. Not really impressed with the service either.
I'm about to use some mainland China airlines next month. I hope they've improved since I last travelled with them in the late eighties. They started serving food after take-off when the plane was still at an angle. Rude too, especially during the domestic flights.
I wonder whether any of the flight attendents wish the passengers were younger and better-looking. When the passengers are old and fat, they don't have much right to complain about the crew's physical appearances.

NorthWorst, as well as employing no air waitresses under the age of 50, also charge $5 for a drink on the Tokyo <-> LA run. You get one free drink with your meal, but all the rest have to be paid for. Last time I flew my colleague had a frequent flier miles card, so I stocked up in the lounge beforehand...
My favorite: Air France. The worst in United Air, especially in San Francisco for some reason. I've dealt with unprovoked hostility from ground crew when I am with my Japanese wife. Racism? Coming from all races though. In January flying out of San Francisco there was this white male flight attendant that was one step from going postal. He was nasty to the Japanese passengers. He was especially foul to another caucasian-Japanese couple, going so far as to tell them that they were looking at him in a fun way. Man! I'm glad that psycho wasn't in our row. I was scared that he was on the plane at all.
I was flying Northwest and accidentally made a loud "pop" noise while opening my bag of peanuts. A flight attendant rushed over and said, "I've only heard that from children!" Some kompanies just tend to tolerate that nasty behavior and it spreads. I think NW is one of those.
I like Aeroflot because it is always an adventure to travel with them, never boring! Many unpredicted events, many good people, great service, especially at Sheremetevo airport and Novotel hotel, the third floor. I want to be there again, because I like the ppliceman at the hotel.

Will NEVER fly a US-based airline EVER. Seems the cabin attendants all got their training at Abu Ghraib prison. BTW - what is it with US airlines and their penchant for hiring grandmas as cabin attendants? I mean, I don't choose an airline solely for the eye candy, but good heavens, last time I flew a US airline with my mother, I thought they were going to recruit her to fill in for someone with arthritis.
SAS.I like self-service there. You can grab food and soft drinks wherever you want. Dislike, they have the oldest stewardess and stewards onboard. Many of them should have already retired. Never seen young and friendly except towards Danish people.

They have really been helpful with me a couple of times as they accepted to change my ticket when that was not possible in theory (changing city of destination and date after the ticket limit), or when I missed my flight (they upgraded me for free on the next flight !). Really, they are making efforts others don't.
Cathay has been the worst for me. At the time of the old Hong-Kong airport, they made us wait a whole day in a small transit room there, no airconditionning, no water, no toilets, no explanations and not one word of apologies.
Best: Singapore, ANA. Of the Brits - Virgin, for relaxed service
Worst: BA - rude, arrogant cabin crew who feel their job is to boss the passengers around rather than serve them (fire them all and replace with nice young Japanese ladies, I say), and baggage handlers who are at best incompetent, at worst malicious. Iberia, Alitalia
