Heat, rice and sugar are a nasty combination.
A lot of these countries are hot year round. The Canadian authors of this report should try jogging, paying tennis or soccer in the hot humid climate where these people live.
In all the developing countries I've visited, the food is really high in rice and sugar, starchy stuff that leads to diabetes.

Hardly rocket science. If you are living in a poor contry, and you can buy a car, computer, and all the toys, you have a lot of money. With the money, you will buy a lot of food. Remember, in some parts of the world a big belly is a sign of status.

It`s not just poor countries. Many working poor families in the US are dependent on their automobiles to get to their jobs and to shopping. Increasingly, only the wealthy can afford to live in walkable parts of cities where cars are unnecessary. This is the result of sprawl and decades of car-centric urban planning.

Scandinavian country historic records revealed something very interesting in regard to longevity. Children born in times of food scarcity (not famine) had longer, healthier lives than babies born to mothers in times of plenty. To me, the take away from that info is pregnant woman should eat a low calorie, high nutrient diet while pregnant, as this indicates it creates a healthier baby. Your info to investigate yourselves, parents to be.
