To add some facts, from around 1900 there had been many accidents worldwide to exploding steam vessels and high pressure containers. This is why in many countries organizations were founded, whose aim is to check high pressure devices against engineering faults. For example the German TUeV (Technischer Ueberwachungsverein) has a branch office in Japan.
Seems that this victim, Tatsumi Akihisa, 34, did not know the basic 101 rules of high pressure:
-make a calculation or simulation of the vessel before testing
-have a safety factor in the design
-have an overpressure release valve
-have a relevant authority cross check the set-up before applying pressure.
This accident was unnecessary. Sorry for the victim, but he also made life for his co-workers dangerous. Not good.
